File Upload and Signature In Survey

This module enhances the Survey functionality by enabling users to upload files and capture signatures directly in survey forms. Multiple file attachments are supported, making it easier to collect detailed responses and approvals.

Single File Upload

Navigate to Surveys > Surveys.

Create a new survey and add your questions.

Set a question type to "Upload File".

Create another question and set question type to "Signature".

Now Start the survey.

In the "Upload File" question, you will find an upload button where you can upload documents.

You can also delete uploaded documents by clicking the "Delete" button.

In the "Signature" type question, you have three options to provide an answer.

You can enter text in the name field to generate an auto-signature.

In the "Draw" tab, you can draw a signature.

In the "Upload" tab, you can upload a file for the signature.

Once the survey is finished, you can review all the answers in the "Review Answers" section.

Multiple File Upload

FOr multiple file upload you need to select "Allow Multiple file upload " option in the question creation.
